Your First Visit
Our friendly dental team wants to make certain that your first visit to Blake Dental Group is as streamlined and pleasant as possible. We invite you to fill out the patient forms below before your first visit. You should expect to spend at least one hour with us at your initial appointment. During this visit, your doctor will ask questions about you and your oral health and will take the time to listen to your concerns and treatment goals.
Your examination will include obtaining a thorough medical and dental history, diagnostic information and x-rays, and a head/neck and oral cancer screening. We will make sure that you understand what we can do together to improve the health, comfort, and esthetics of your mouth and will provide a treatment plan that you can understand and feel comfortable with. In some cases and where appropriate to the treatment plan developed for your needs, we may be able to perform a cleaning at your initial visit.
Comprehensive Dental Care
45 min – 1.5 hours
Your appointment at Blake Dental commences with a thorough assessment of your teeth and gums. Employing a collaborative co-diagnostic approach, we craft a personalized treatment strategy tailored to your specific objectives and requirements. Your evaluation encompasses the use of low-radiation digital X-rays, intraoral images, and an oral cancer screening. Subsequently, we delicately remove plaque and tartar situated beneath your gumline. Our session concludes with the development of a customized home care regimen, meticulously designed to cater to the unique needs of your oral health, facilitating the maintenance of your radiant smile between appointments.
Dental Emergency
30 minutes or longer
If you experience a dental emergency, kindly contact our office, and we will make every effort to promptly accommodate you and alleviate your pain. Upon your arrival, we will conduct a thorough clinical examination lasting 15-30 minutes and capture an X-ray of the affected region to assess your condition accurately. Following this assessment, we will present a detailed treatment plan to restore your comfort.

We will happily work with you to help you sort out your insurance coverage, and also help you navigate your financial options for service. We proudly accept plans from Delta Dental and GEHA Connection Dental.
We recommend contacting our office before your visit so we can gather your insurance information and verify benefits. We will pass long an explanation of your dental benefits and your financial responsibilities, and work together to ensure financing—if need be.
